You need to fill out an application form and submit all required forms in order to apply Dove Science Academy. At least one adult (parent / guardian) should sign the application form. Students will not be admitted unless we get all required documents.

For 6th grade registration for 2011-2012, please complete the registration by April 8, 2011. After April 8, we will accept students from waiting list for those who did not complete the registration.

If you complete your application online, you need to download other required forms and fax them to our school or fill them out in our school office.
